What is a DA Form 1380?

DA Form 1380, Record of Individual Performance of Reserve Duty Training, documents duty that Army Reserve members complete outside their normal unit drills, such as nonunit training, professional courses, or attached duty. Units use it to record and certify drills, Annual Training, and other duty assignments. This record matters because it confirms the training performed, which affects pay and benefits, and ensures reservists receive accurate retirement points and credit toward promotion.

When is a DA Form 1380 not required?

You can skip the form when duty falls outside nonunit Reserve training. Regular active duty soldiers don't need it, and neither do reservists enrolled in Army Extension Courses or simply signing the Inactive Duty Training attendance roster during authorized paid assemblies. Training performed with a reservist's own unit of assignment also falls outside its scope.

Frequently asked questions

An officer with direct knowledge of your duties must sign the form. This person verifies that the duties listed in Item 11 actually took place. Depending on the type of training, that officer could be your unit commander, your attachment's commanding officer, or the chief of the proponent agency overseeing the duty.

Draw a line through the wrong entry and write in the correct information next to it. The officer who verified your duties needs to review the change and sign again to confirm it's accurate. Once that's done, send the corrected form back through your chain of command so it can be processed without delay.

There's no official rule that sets a fixed number of years for keeping this form. Even so, hold on to your own copy after you submit it. The form records retirement point credit, and you may need to show it decades later when your retirement points are calculated.
